FACILITY RENTAL AGREEMENT

 

All persons or organizations desiring to use the CCUMC facilities are required to complete this Facility Rental Agreement form.  All renters are required to observe all rules and regulations.  Failure to comply may result in the denial of future requests.

PLEASE NOTE:  The renting group/individual will be responsible for arranging the tables and chairs to their own particular need.  Please, do not drag tables across the floor.  Return tables and chairs to arrangement you found them.  (Suggest taking a picture, if able, of arrangement as you find it.)

 

* The renter agrees to pay for any damage or breakage to church property.

* The renter agrees to provide full and adequate supervision of all members of the renting group.  The renter agrees to leave the premises and building in good order.

* All personal church properties are to remain on the premises, including tables and chairs.  The Fellowship Hall may be used for community activities whose purposes are in line with the purposes listed in our United Methodist Book of Discipline, and not  commercial enterprise.

* We do not allow alcohol or other drugs on church property; and only allow tobacco use in the designated area described as twenty feet east of the exterior fellowship hall doors on the east side of the building.

* It is our custom to run the kitchen according to State regulations which includes among other things: requiring covering of hair, wearing of gloves, and shoes to be worn in the kitchen.  Caterers must be state licensed and provide proof of liability insurance.  See attached sheet of Rules and Information.

* Events need prior approval by Trustees.

* Use of any kitchen equipment (freezer and cooler are excluded) is limited to church groups, church functions and church family, unless under the supervision of the Kitchen Coordinator.  All other renters are required to furnish their own equipment such as coffee pots, roaster, warmers, etc.  All renters must purchase their own paper products such as paper towels, napkins, place mats, table coverings, cups, cream/sugar, with the exception of aprons, hair covers, gloves which will be provided by the church.

* Do not leave any food on the premises.

* On any evening, the renter agrees to leave the building by eleven p.m. after completion of clean-up.  Any exception needs prior approval by the Trustees.

* Not responsible for lost items or items left on premises.

* NO balloons allowed.

 

 

The individual or group signing this agreement as a renter accepts full responsibility for payment of all rental charges and labor costs arising from the use of church personnel and facilities.

FACILITY USE SCHEDULE OF CHARGES

 

Note:  A $50.00 deposit is required to secure a date on the calendar. 

The balance of all fees must be paid a minimum of two weeks prior to the event.

 

Fees for PARTICIPANTS

(A participant has been defined as a person in attendance or giving financial support in the last six months. Charges listed for a Participant apply when use is for the participant themselves, their spouse, parent or child.  All other relationships fall under the non-participant fee schedule below.)

There is no facilities charge for a funeral for a participant.  Meal arrangements

are to made with the United Methodist Women.

          * $50.00 refundable if everything (including restrooms) is left in satisfactory

          condition as determined by the Kitchen Coordinator and/or Trustee.

 

          Use of Full Kitchen

          Not available to non-participants.  Used only at discretion and permission of

          Trustees)

                    

          Funerals for Non-Participants

          Fellowship Hall charge for the use for a funeral for a    non-participant.….. $150.00

          Meal arrangements are to made with the United Methodist Women.

 

Notes:  ***Groups charging a fee for attendance will be charged regular fees.

             *** Occasional use allowed by non-profit groups at the discretion of the Trustees.

RULES AND INFORMATION SHEET

 

Rules to follow and information you should know before using the kitchen.

 

HEALTH: Do not use the kitchen if you are not in good health

 

SANITARY ATTIRE: Hair nets or appropriate hair coverings, gloves, and aprons must be worn at all times while in the kitchen.

 

HOT WATER heater is to be turned to 140 degrees. This is necessary for both the dishwasher and the pot and pan sink.

         

DISHWASHER:

(note: this will be re-write and include pictures)

 

STOVE: The fan must be turned on when using the stoves. There are two convection ovens - must be turned on if you want to use them as convection ovens.

 

POT AND PAN SINK:

          * hot water must be used in all three sinks.

          * the third sink must have bleach added according to directions on the wall. please check with

 the strips. (50%)

         

FOOD TEMPERATURES: see instruction sheet

* hot food 140 degrees or hotter

          * cold food 45 degrees or colder

 

NO SUPPLIES/EQUIPMENT WILL LEAVE THE CHURCH.
